If you’re a social worker, nurse or other medical professional who needs treatment for a patient, we can help. We provide service to homebound patients as well as to those who are appropriate for outpatient. We know your time is limited. Our goal is to make the referral process as easy as possible for you. To fax a referral, include patient demographics and a copy of their insurance cards (front & back) and driver’s license to 214-614-9352.
Outpatient
We are an outpatient clinic in-network with Medicare, Blue Cross Blue Shield of Texas, United Healthcare & Baylor Scott & White. Visit www.LTStherapy.com/patients for insurance updates. We accept all other carriers and bill insurance on behalf of patients. Visits are done in the Little Elm office. Home visits may be made within a 10-mile radius on a case-by-case basis. A patient cannot be on service with a home health agency (whether seen in office or in the home) and receive our services at the same time (unless they choose to pay privately).
Home Health
If your patient is on service with a home health agency, we cannot see them at the same time except in two cases. One: A patient is paying cash for their care (or sometimes in a hospice situation). Two: We negotiate a contract with a patient’s home health agency (or use one with whom we partner).
Note: As of 2021, we have suspended home health agency partnerships.
